Functional rehabilitation

Occupational therapists work with clients with a range of difficulties caused by a variety of conditions or incidents. Whether it be following a heart attack, surgery, orthopedic trauma or just the challenges of older age, our occupational therapy team will work with individuals to provide rehabilitation programmes, activities or techniques to help restore their previous level of function or adapt to their new one.

What is functional rehabilitation?

We refer to functional rehabilitation as the process of working towards returning to an individuals maximum level of independence in the activities that are important to them. A person’s priority of their functional activities are as varied as the individual to whom they apply. For example some people will wish to focus on restoring their ability to live in their home independently with activities such as bathing and preparing meals. Whereas others will be looking to restore function in getting back to the gym, college or community activities.

Through a collaborative assessment process our OT and the client will identify the areas of functional difficulty in day to day life to focus a rehabilitation programme on. This can be focusing on one activity for a short period of time such as bathing or it may be a more comprehensive programme to ensure that maximum levels of independence are reached across all areas of day to day life.

Our approach

We aim to provide relevant, timely, goal orientated rehabilitation programmes to clients in their own homes and communities. Our collaborative approach with the client at the centre involves working with other professionals (case managers, therapists) family members and carers to ensure rehabilitation is focused on making a practical improvements. Our aim is to help clients and their families to become as independent as possible, as soon as possible, through result driven occupational therapy delivered by expert therapists.

Packages of care

Assessment & advice

An initial assessment is carried out in a client’s own home to establish difficulties and create a treatment plan that works for their needs, lifestyle and budgets. We adopt a consultative approach to gather information and provide any immediate verbal advice where we can. The assessment is then followed up with a detailed report including specific recommendations.

Treatment

Bespoke treatment packages are delivered through one to one sessions working towards functional goals that are realistic and achievable. Treatment is tailored around the functional challenges that are identified during the assessment.

Examples of rehabilitation treatment programmes may be weekly sessions to boost confidence at home after a hospital discharge or several times per week with the aim of regaining mobility in the community. Sessions are delivered for as long as needed in blocks of 8 to allow us to review progress and goals as we go.

Home programmes

Not all clients will need one to one treatment sessions.

A home programme that is designed by us but carried out either independently or with family support can provide rehabilitation techniques and strategies to be implemented into every day life. A home activity programme has the advantage it can be completed when energy levels and time allows.

Fatigue management

Fatigue is a common difficulty associated with the recovery and adjustment phase of accident, illness or injury.

We specialise in bespoke fatigue management programmes with guidance and support to enable the management of fatigue in daily life. The package is often 8 weeks of one to one sessions where we provide strategies and techniques to minimise the impact of fatigue on rehabilitation.

Falls prevention

Falling over is a major cause of disability in the UK, with around half of falls resulting in significant injury leading to reduced confidence, social isolation and increased fragility.

However, many falls are easily preventable with a few adjustments around the home, along with physical activity to increase strength and balance.

The OT Practice offers an eight-week Falls Prevention Programme, all conducted within the home, to reduce the risk of falling while increasing mobility and confidence, including:

  • A full initial assessment of physical aptitude, domestic environment and mobility in and out-of-doors.
  • Recommendations for home adaptations to reduce the risk of falls (installation can be arranged if desired).
  • An individualised, illustrated exercise programme to help increase strength, balance and flexibility.
  • Guidance and practical demonstrations to help you cope in the event that a fall does occur.
  • Advice to family and carers.
  • Thorough and detailed reports will be provided after each assessment.

Training

Training to family, carers or support workers can compliment our other treatment packages or sit alone as a solution to an individuals rehabilitation needs.

Rehabilitation training packages are tailored to the needs and audience of each case and delivered in the environment that they will be working in.

International services

A select team of our rehabilitation experts travel overseas to provide one to one specialist assessment and treatment where needed.

Whether for an intensive block of rehabilitation or to set up a programme with carers, our carefully selected international occupational therapy team provide a discrete service with the highest levels of clinical care.

My son / daughter has difficulty with motivation, how can an occupational therapist help?

Motivation is our reason or reasons for acting or behaving in a particular way. It is a crucial element in setting and attaining goals or aspirations and any reduction or difficulties in motivation can affect a person throughout all aspects of their life.

An occupational therapist would complete a full assessment with your son / daughter and seek to identify the reasoning for his reduced motivation. Within this assessment the therapist would explore all aspects of his life including his ability to complete self-care tasks, engage in productivity and interact with his leisure activities. They would then seek to explore your son's / daughter’s interests and hobbies and potentially use these activities within a bespoke treatment plan with the aim of improving their ability to complete everyday tasks.

The activities used within this treatment plan will vary from person to person and will be specific to identified needs and the levels of restrictions.

Extreme tiredness and fatigue are stopping me achieving, how can I manage it better?

There could be many reasons for your tiredness and fatigue which can negatively impact on performance throughout all aspects of your life. Some of the reasons behind tiredness and fatigue include psychological factors such as stress, anxiety or depression. Physical factors such as anaemia, diabetes or glandular fever may also be having an impact. In addition to this, you could also be experiencing physiological reasons including pregnancy, breastfeeding, inadequate rest or sleep and excessive exercise.

In order to help you to manage your fatigue level it is necessary to understand your lifestyle. An expert occupational therapist would be able to complete a full and in-depth assessment of your health and social needs to determine the effect of these on your fatigue and tiredness levels. Through gaining this information they would be able to formulate a bespoke treatment plan and advise you on which areas of your lifestyle you can change independently and which areas may require more specialist intervention.

I am having difficulty moving around and accessing my home, what can an occupational therapist do to help?

Our specialist occupational therapists would visit you at home to see your physical environment and observe the challenges you are experiencing.

The OT will to look at all aspects that can affect your ability to complete activities within your home, this includes access and the tasks you need and want to complete on a regular basis. This knowledge of your health and social situation will enable us to advise how best you might simplify or adapt your environment to meet your needs and minimise any risks.

All of our recommendations are based on your assessment and are bespoke to you and your home.

We specialise in this area of occupational therapy and have developed numerous professional relationships with contractors and adaptation specialists who can implement our recommendations in a timely, professional and sensitive manner, keeping you fully involved throughout the entire process.

My mother / father can’t stand for long enough to cook a meal, what can be done to help?

This is a common issue and could be for a number of reasons. There are many aids/adaptations that can be incorporated to facilitate your mother / father’s independence with meal preparation and prolong their standing tolerance.

Our expert occupational therapists, using their specialist knowledge and skills would determine the health and social needs you are looking to address. In doing so we would also gain a greater, more individualised understanding of the challenges your mother / father is experiencing. The collection of this individualised knowledge and understanding will enable us to develop, together with you and your mother /father, a specific programme of treatment and / or advice with the aim of prolonging standing tolerance during meal preparation.

There are many ways in which an experienced occupational therapist can help boost confidence and facilitate the development of routine following a period in hospital or respite care.

Our expert occupational therapists would visit you in a place that is comfortable to you and explore what your routine consisted of prior to your time in hospital or respite care. We would also explore your current health and social needs and identify the areas or challenges that you would specifically like to address.

Yes - we are able to provide treatment sessions based on a hospital discharge report. Our office based client manager, who is also an experienced occupational therapist will discuss this with you at the enquiry stage and will be happy to review the report for you at this stage. If we feel that your mother / father would benefit from a fresh assessment and review then we can advise of this at this stage.
