Neurological rehabilitation

Brain and spinal injuries can be life changing. Our team of expert neurological occupational therapists support the rehabilitation process helping those affected and their families find their new version of normal. For some it will be to support a hospital discharge, adapt a new property and sourcing relevant equipment to maximise independence. For others we can work on community activities, daily routine, memory or fatigue management, cognitive function or returning to work.

What is neurological rehabilitation?

Neurological rehabilitation under the care of an occupational therapist will consist of supporting and facilitating an individual to be as independent as possible in their daily life. For some this may involve focusing on improving reduced cognitive functions such as planning or memory, whereas for others it may be more focused on removing physical barriers to independence through well designed housing, assistive technology or equipment. Intervention is always goal orientated and focused around maximising and enabling independence, no matter what form this may take.

Our approach

We aim to provide relevant, timely, goal orientated rehabilitation programmes to clients in their own homes and communities. Our collaborative approach with the client at the centre involves working with other professionals (case managers, therapists) family members and carers to ensure rehabilitation is focused on making practical improvements. Our aim is to help clients and their families to become as independent as possible, as soon as possible, through result driven occupational therapy delivered by expert therapists.

Packages of care

Assessment & advice

An initial assessment is carried out in a client’s own home or rehabilitation unit to establish current challenges and goals allowing us to formulate recommendations for rehabilitation.

If a client is in a rehabilitation unit we can also visit their home or proposed new property to advise on adaptations and equipment needs.

We adopt a consultative approach to gather information from the MDT, family and carers ensuring recommendations are practical, in-line with expectations and within budget.

Treatment

Bespoke treatment packages are delivered through one to one sessions working towards functional goals that are realistic and achievable.

Treatment is tailored around the functional challenges that are identified during the assessment. Examples of neurological rehabilitation treatment programmes may be cognitive/memory strategies and development, community access, preparation and support with returning to work or voluntary work opportunities.

Sessions are delivered for as long as needed in blocks of 8 to allow us to review progress and goals as we go.

Home programmes

Not all clients will need one to one treatment sessions.

A home programme that is designed by us but carried out either independently or with family support can provide rehabilitation techniques and strategies to be implemented into every day life. A home activity programme has the advantage it can be completed when energy levels and time allows.

Falls Prevention

Falling over is a major cause of disability in the UK, with around half of falls resulting in significant injury leading to reduced confidence, social isolation and increased fragility.

However, many falls are easily preventable with a few adjustments around the home, along with physical activity to increase strength and balance.

The OT Practice offers an eight-week Falls Prevention Programme, all conducted within the home, to reduce the risk of falling while increasing mobility and confidence, including:

  • A full initial assessment of physical aptitude, domestic environment and mobility in and out-of-doors
  • Recommendations for home adaptations to reduce the risk of falls (installation can be arranged if desired)
  • An individualised, illustrated exercise programme to help increase strength, balance and flexibility
  • Guidance and practical demonstrations to help you cope in the event that a fall does occur
  • Advice to family and carers
  • Thorough and detailed reports will be provided after each assessment.

Fatigue management

Fatigue is a common difficulty associated with the recovery and adjustment phase of accident, illness or injury.

We specialise in bespoke neurological fatigue management programmes with guidance and support to enable the management of fatigue in daily life. The package is often 8 weeks of one to one sessions where we provide strategies and techniques to minimise the impact of fatigue on rehabilitation.

Training

Training to family, carers or support workers can compliment our other treatment packages or sit alone as a solution to an individuals rehabilitation needs.

Rehabilitation training packages are tailored to the needs and audience of each case and delivered in the environment that they will be working in.

Property reviews

Our team work with families or case managers to review proposed properties in order to assess them for suitability based on the client's current and likely future needs.

Once we have assessed the client and reviewed the properties we will be able to advise on appropriateness for adaptations, space for equipment, access and any other relevant factors to support the decision process for purchase or rental.

International services

A select team of our neurological rehabilitation experts travel overseas to provide one-to-one specialist assessment and treatment where needed.

Whether for an intensive block of rehabilitation or to set up a programme with carers, our carefully selected international occupational therapy team provide a discrete service with the highest levels of clinical care.

Questions about difficulties that our OT's help with

My son has hidden difficulties after his brain injury such as memory and concentration, how can an occupational therapist help him improve these?

An experienced occupational therapist would be able to implement a variety of techniques and strategies to assist your son to improve his memory and concentration.

In order for them to do this they would need to have a discussion with you and your son to explore what his memory and concentration was like prior to the brain injury, the injury itself and how this is affecting currently. The OT is likely to use some standardised assessments to identify where these difficulties are coming from and what day to day circumstances they are likely to be seen in.

Some of the activities that our occupational therapists may implement include games that require a person to use their concentration and/or memory skills e.g. word searches, card games. By using these activities in a graded and therapeutic manner they will develop and improve these skills and be able to transfer them into day to day live both at home, school/work. The treatment plans and activities used will vary from person to person and will be in response to specific needs and level and type of restriction.

Why am I struggling to plan my day or an every day activity such a making a meal after a brain injury and how can an occupational therapist help?

To initiate and organise a plan of action, be it for one activity or a full day, you need to be able to:

  • Think ahead.
  • Concentrate.
  • Remember things.
  • Gather and process information.
  • Prioritise.

If you have experienced a brain injury, particularly of you have damaged the front of your brain you could have difficulties with your initiation, planning and organisation skills. This could explain your difficulties.

An occupational therapist can work with you to identify what the daily challenges are and why you are experiencing them. They can then work with you to provide practical advice and support to implement strategies to move forwards. Examples could include:

  • Assisting you to minimise distractions.
  • Development of a simple set programme and routine.
  • The use of diaries and other assistive technologies.

The treatment plans and strategies/activities used will vary from person to person and will be in response to your specific needs and challenges.

How can an occupational therapist help with fatigue?

Fatigue is described as a subjective feeling of tiredness which is distinct from weakness and has a gradual onset. Fatigue can have both a physical or mental cause and can generally be alleviated by periods of rest.

Our experienced occupational therapists would explore the suspected reasoning for fatigue and implement a number of techniques and strategies to assist in its management and reduction.

In order to assist a client with how best to manage their fatigue, it is necessary to understand their lifestyle. An expert occupational therapist would be able to complete a full and in-depth assessment to determine the effect of lifestyle on your fatigue levels. They will then provide practical and achievable recommendations in order to provide you with the skills and support you require to move forwards.

What kind of equipment is available to help maintain independence following a spinal injury?

There are many different types of equipment that is available to help maximise independence following a spinal injury. Some can be used on a temporary basis during the rehabilitation stage, where other items can become a permanent aid to independence and safely.

The types of equipment that would help maintain independence depends on the type and significance of the spinal injury and can include smaller items such as lever taps, perching stools, bath seats or larger, more complex equipment such as wheelchairs, ramps, internal lifts, adapted vehicles, fully adapted bathrooms or kitchens.

Following an assessment our occupational therapist will provide a full list of proposed suitable equipment and suppliers. We are able to assist with arranging trial as required to ensure that you purchase the most suitable equipment for your needs and lifestyle.

I need to make some modifications to my home, how can an occupational therapist help?

An occupational therapist can advise on suitable alterations to your home to maximise your safety and independence. Our specialist occupational therapists would visit you at home to assess how your environment can best support your needs.

Our occupational therapists will seek to look at all aspects that can affect your ability to complete activities within your home including:

  • Access in, out and around your home.
  • Activities completed within the bathroom such as transferring in and out of the bath, on and off the toilet, standing at the basin for teeth cleaning, washing or shaving.
  • The kitchen and if there are any challenges due to the current design and layout.

They can then provide practical and achievable recommendations on any alterations that can reduce risk and make your daily life easier.

Suggestions maybe:

  • Minor such as the removal of rugs to reduce falls, the installation of grab rails or some aids for the kitchen.
  • More significant such as stair lifts, removal of the bath and the installation of a level access shower or wet room for example.

I can not access my bath and need to adapt my bathroom, what advice can you provide?

Successfully negotiating getting into and out of the bath, requires a high level of both upper and lower limb muscle strength, balance and stamina. It is one of the most common challenges associated with a change in mobility levels regardless of the cause.

An occupational therapist can work with you to assess this activity and provide any short term / small changes that may assist if you would prefer not to remove your bath initially. These may include; the use of battery operated bath lifts, teaching you different transfer techniques or the installation of grab rails.

However if the occupational therapist felt you were at risk completing this transfer, if you have a deteriorating condition or just wish to move to showering straight away, they will be able to advise on the most appropriate solution to meet your needs.

There are many considerations to be made when adapting your bathroom to ensure that it meets your immediate and also longer term needs, for example:

  • Whether to move to a wet room style shower area or a level access option.
  • Do you require a seat in the new shower area and if so what style will best suit your immediate and long term needs?
  • Do you require grab rails located in the shower area and if so where?
  • Are you going to be showering independently or will you require a carer to support you, either currently or in the future?
  • Who else is going to be using the space?
  • Do you have any sensory impairments such as reduced sensation, visual or hearing loss?
  • If you have a history of falls, seizures or an unstable medical condition how will you ensure help is available if required?

We also recognise the importance of aesthetics in bathroom design and recognise the importance of a bathroom being a beautiful, relaxing and functional space for you and your family.

An occupational therapist will complete a full assessment with you to identify a bespoke specification for your bathroom. We work with nationally placed bathroom companies that we can put you in touch with to ensure your bathroom is installed by a trusted and experienced contractor.

How can an occupational therapist help with community rehabilitation following a brain injury?

There are many different ways an occupational therapist can assist with community rehabilitation following a brain injury. The methods, techniques, services and strategies offered depend on the severity of injury and the challenges they are experiencing in day to day life as a result.

Our experienced occupational therapists would visit a client at home or in a rehabilitation unit prior to discharge, to assess their previous and current levels of mobility and cognitive ability, their goals for rehabilitation and their progress and rehabilitation journey to date. From here we can create a goal orientated, practical and realistic rehabilitation programme.

My son has a spinal injury and is due to be discharged from a rehabilitation unit. I want to pay for some additional rehabilitation, can you help?

Yes - We are able to provide a comprehensive and intensive period of home based rehabilitation for as long as your son was making progress and desired our involvement. Ideally we would visit your son in the current rehabilitation unit to liaise with his therapy and medical team, this ensures we can compliment their work so far. We would also use this as an opportunity to assess your son’s current levels of mobility and identify his challenges.

Once we had met your son, it would be helpful for us to assess the home environment prior to his discharge. This allows us to work with the current therapy team to ensure that there is the appropriate equipment in place. Home adaptations can take some time to be installed and so depending on the level of injury we may look at instigating these at this stage also.

Once your son is home we would work with him to set realistic and achievable goals for rehabilitation and establish a therapy routine. Goals are unique for each person and often smaller goals are set along the way to achieving a more significant milestone including:

  • Learning to use environmental technologies to control his environment including: using his telephone, selecting music or the television, controlling lights, opening and closing the front door to allow access for example.
  • Looking at the return to old or identifying new hobbies such as wheelchair sports.
  • Vocational activities should they wish to return to voluntary or paid employment.
  • Maximising opportunities for social engagement.
  • Driving or being able to access a vehicle.
  • Maximising independence with showering and dressing.

How can I maintain my independence with a diagnosis of Huntington’s disease?

Occupational therapists can help people with the challenges of huntington's disease in a number of ways.

Through a thorough and collaborative assessment with you and your family, we will identify the current areas of difficulty and plan for the challenges you are likely to experience in the future.

Our experienced occupational therapists can help implement strategies to minimise and manage fatigue symptoms, make reasonable adjustments to work and daily routines, as well as offer the provision of suitable daily living aids, equipment and adaptations.

My father has Parkinson’s disease and wants to remain at home, how can an occupational therapist help?

Our specialist occupational therapists would visit your father at home to assess his home environment and how he moves around it. This would include looking at all areas of daily living within the home from when he gets up in the morning to when he goes to bed in the evening.

This assessment allows us to identify any potential risks, the challenges he is currently experiencing and likely ones for the future, his daily routine and goals for remaining as independent as possible.

With this information we can then provide practical and achievable advice on how to maximise his independence for as long as possible. Recommendations are likely to include:

  • Small changes to the home environment to reduce the risk of falling (removal of rugs and clutter for example).
  • Small items of equipment to allow him to make and move meals and drinks around the ground floor safely.
  • An appropriate seat and bed to reduce the challenge of rising from sitting or lying.
  • Teaching of techniques to reduce the episodes of ‘freezing’ often associated with parkinson’s disease.
  • Advice on alternatives to using the stairs for when this becomes challenging.
  • Bathroom adaptations to maximise independence for as long as possible and then to ensure he can be supported by carers with minimal risk and maximum dignity.
  • Looking at access to and from the home i.e. ramping.
  • Advice on outdoor mobility and if a wheelchair would be of benefit we can advise on this.
